Consult the evaluation questionnaires.

Click on the plus sign at the right.

Enter a name for your evaluation questionnaire.

Enter a description of your evaluation questionnaire.

Select whether or not it is used by the admin.

Select whether or not it is used by the collaborator.

Select whether or not it is used by the customer.

Click on 'Create evaluation questionnaire'.

Create or modify some evaluation questions for your questionnaire.

Click on the plus sign at the right on the page of your questionnaire.

Select the evaluation question of your choice.

Repeat for the remaining questions.
